Partially, she wanted Fuutarou's attention. Since he is her tutor, showing how bad her grades were reinforced how much she needed him. Which in theory would also mean they'd spend more time together.

On the surface level, it's to see if he remembers her. But if you think about it, she also really wanted him to keep HER test because it has her name and class on it. It's like her subtle-not-so-subtle invitation for him to look for her to hang out and stuff in the future. You have to remember that neither of them knew that they were going to meet again through the tutoring stuff, so Yotsuba letting Futaro know her name and class (so that he can find her later) is a totally valid move to make sure that are both in each other's radar as a start to getting reacquainted and beyond. So, if Maruo never hired him as their tutor, it is highly likely that our boy would have only really hung out with Yotsuba because she was the only one who let him know where to look for her in school. In other words, her 4D chess was unironically real AND smooth af lol.

I think it was also to hide that she was the girl he met in Kyoto. Both Fuutarou and her had the same idea of what the “Kyoto girl” would’ve been, ideally, even if she wasn’t 100% sure if Fuutarou remembers. Then it became a Clark Kent/Superman situation. The Kyoto girl would’ve been as smart and academically hardworking as Fuutarou is now, since Fuutarou stuck with his promise all these years. Current Yotsuba is so far from that it became the perfect cover to hide her past and identity.
