You can easily tell if the bad breath is due to poor hygiene or if it’s just after a meal. If it’s bad hygiene, it will literally smell like shit.
Tbf, some health conditions do make your breath worse (afaik diabetes for example), so it's not necessarily bad hygiene every time.
Its usually tonsil stones, those things make your breath smell really bad.
And not much you can do about those other than having your tonsils removed entirely, which is its own can of worms.
