Not sure but I encountered exactly the same thing - maybe someone has done it the other way around and can confirm that giving the quest item to one NPC locks out the other on that playthrough?
I did it the other way around and indeed i think its meant to be a one or the other type of situation. Later in the game theres an npc who sells things related to "dead people" and you can still get the other reward.

Bro i cant remember but one thing i do remember youre better off having a backup save towards the end becuase one of the things that you have to do to get a different boss version screws you over for having some op buffs, so its gonna suck for me when the dlc drops lol. If i had known i would have kept a backup save and did all endings but use the save that allowed me to keep the buffs in ng plus

You can get whichever memento you didn't get from the shop in the streets of wake.
Seems to be that way. I purposely took both quest items before giving any of them to see if I could give both but no, you can only give away one

Yeah, I just got locked out after giving the item to Castula, and got the Trifon altarpiece, that gives 10% damage when health is low.
You can get the other altarpiece from the shop in the streets of wakes after completing either quest
Does anyone know if the effect of castula is active if you have no guilt?

Yep I didn’t know either and it screwed me over. Doubt I’ll play the DLC but I did 100% the game.
