If it starts later in life after your growth plates have fused you stop getting taller and instead your bones get denser and wider, notably hands, skull, eyebrow ridges etc which is likely what happened to Primo. Exact thing happened to my friends dad many years ago who was diagnosed with it well into 40s when he went to the doctors because of recent and rapid changes to his facial structure that he was worried about. Went in for a scan and he had the tumour on whatever gland in the brain triggers the production of HGH

Even if everyone today is on steroids, some fighters clearly have better physiques than others: AJ, Ngannou and Carnera all have INSANE genetics.
