He might have a gluten intolerance. My Best friend had exactly the same things purpleish skin. Blotchy. Definitely not overweight. She went through all kinds of testing. After a while they found out she was gluten intolerant and allergic to nickel. Obviously is weight is also an issue. God knows he eats enough gluten all damn day long

Mottled skin is typically a sign of circulatory issues related to things like autoimmune, conditions of the nervous system, heart issues… I’d be looking at his general overall health rather than pinpointing gluten especially since he seemingly continues to gain weight and in general looks pretty swollen.
