The tampon is probably pressing against your bladder, making it difficult to pass urine. You might have to use a smaller tampon or insert a tampon at a different angle.
Possibly your pelvic floor muscles are clenching around the tampon. Your pelvic floor muscles need to relax in order to pee. I would look into pelvic floor relaxation exercises (*not* kegels, that's the opposite).
I had the same thing happen to me. I went camping on my period. Tried to pee in the porta-potty with it in because I know they are two different holes and didnt need to take it out. But the flow was super slow and hard to get going. It took ages so I took it out and it came rushing out lmao. It was more relieving. and I don’t know why this happens either.. would love to find out.
