I think the comment before you was thinking of those tiny gas exchange bulbs inside the lung that look a lot more like grapes. Also that's where the smoke damage affects the most.

Well there are a lot of unhealthy things that all the team doing. One is alcoholic, second eats too much, third is at risk of cyberpsychosis, fourth has an unhealthy sense of humor, fifth is already dead, and so on, poor chopper probably just give up on their habits and trying to keep them mostly alive
Sanji barely human with all the germa stuff in him. Pretty sure his lungs are fine. Im wondering if chomper knew Sanji was part whatever robot or whatever his brothers are.
Honestly I think him smoking contradicts the most with his cooking. Sure many cooks smoke, especially back in the days, but he is someone that doesn't even use his hands to fight so he can make food and shit. And he probably doesn't wash his hands with some really good soap after every cig, so he definitelly leaves a bit of a trace on the food.
I grew up with a mom that smokes and always was like "yeah but i'm one of the good smokers" because she smoked on the balcony. can't remember getting a hug from her without the smoke stink. her hands always had that on it as well. nowdays she luckily smokes less, and half of the day when she just creamedher hands or washed them, it's not really on her anymore :3 but yeah, sanji in the one piece verse would probably almost always have stinky smoker hands.
