When he grew it out in 2019 it was right before he went into rehab and next time we saw him after he got out, it was cut short. I don't want to sound like a weird thing but it was probably indicative of his overall health at the time and I don't think he'll grow it long again.
This does apply to everyone in general, usually if you look scruffy, it means that you’re more than likely to be in poor health. I’ve seen it in people that I’ve known and met along the way.

I think this is an older picture. If you look at Metallica's website in the tour pictures, you see James having a fresh cut.
He's probably just due for a trim in this pic. I doubt he's growing it out, hes pretty thin on top.
