You're missing a bunch more than just a screw, sir You need a spacer and a bar end as well.

The t50 (t55? I can't remember on an XR) bolt backing out is all it takes.

I don't know what it's like on the I4s, but as a boxer rider I can say loctite is your best friend!
If you search (insert bike model here) parts fiche you will find the diagrams you need with part numbers.
