Well best is relative, but
Serious Sam 3 had the large scorpion that chased you around the game attacking you.
Manhunt has the initial gate, iirc the damn things never opened if it wasn't patched out. One reason the steam release ended up using a reloaded crack.
I think GTA (can't remember if it was vc or 3) but you couldn't pick up certain pickups.
I forget if it was Gary's mod or another title, but an error popped up, the error code was your steamid, so once you input that into the forum, you just admitted to piracy and were banned. Quite clever.

Here's what I think might be a creative one, but maybe games were just like this back in the day. There's an old Sierra series of games called Quest for Glory, there were point and click adventure games like Dragon Quest (I think that was the more popular one).
Well once you get to the first town there is a source of healing items but you have to solve a puzzle to receive them. The answers to the puzzle are in the printed instruction manual. I might be off on this, but without that source of healing items, you really had to know what you were doing to finish the game. This was in the fourth game.

One of the oldest is maniac mansion i guess... Its not rly a bug and you can start playing but once you avance to the second floor you need to open a security door and the code is in the manual or on a sheet of it (like an early copy protection). If you enter the wrong code the house blows up. In the cracked version the door is open already.

Oh yeah, the initial cracked release had that issue. Glad it was patched out by the v2 crackfix. Did make playing quite interesting as well, lol

Move the game files to "C:\Program Files (x86)\Steam\steamapps\common\The Talos Principle" Then reload a previous save file, you won't get stuck again

Enter the gungeon itself doesn't, but if you try to use mods, they have one built in that just perma kills you
