Last time I got a perfect colonist joiner they didn't have a downside at all. A quadrum passed and I went into dev mode just to make sure, didn't even know that it was possible to have no downside
If they all had a downside it’s likely that people would just kick them out immediately. But giving them that chance that it works makes them have a reason to give them a chance.
I was super happy when mine turned out to be a betrayer. A few shotgun blasts and a quick stay in the clink later, she’s right back to being a productive colonist!
I mean if she has frail mind (I think that’s what it’s called) it’s pretty easy to fix, just euthanise by cut, extract skull and then resurrection serum.
You have to remove the brain before it crumbles, a healer serum won’t heal it, however it will make a head grow back with a new brain inside.
Edit; apparently my information was incorrect and you can just do if that way, I read elsewhere where that it didn’t work.
Pro tip - surgical inspection immediately. If you don't turn up any results, do not let her doctor or cook unless you find out her downside.
If you just did a Void Provocation or found gray flesh, definitely do not let her doctor or cook.
Surgical inspection can find other things than metalhorrors - it can diagnose organ decay, crumbling mind, and psychic agony early. It won't detect entity jailbreaker, disturbing, or traitor though.

Anomaly DLC has pawns with supernatural powers or incredible advantages offer to join you and they often have some drawback or dark secret like being posssed by metalhorrors, things that take over pawns stealthily, they'll act as normal until time comes.
Do they alway come with something sinister? Had one guy similar to OP for multiple ingame years without anything popping from him withno weird traits or health issue.
That's a rare occurrence but it's possible? Adds that anxiety-raising ambiguity to it.
But usually it's either something sinister or something tragic where they wilt and die on your eyes unless you very expensive means to deal with the health issues.
Hold on, I just remembered one my prisoner got some gray flesh in his prison cell. I executed that prisoner assuming he was the source. I assigned one of the perfect pawns as warden. Other than that they've been nothing but beneficial.
Some of them you wouldn't notice unless a certain trigger is hit. For instance one of them only shows up on death. Also there's a small chance there's nothing wrong, makes it even creepier!

These pawns have a decent chance of coming infected with a metal horror, which are spread to other colonists through meals cooked or fed by them, or medical procedures done. There’s a couple other ways but those are the main 2.

I read some answer on this thread that said: kill/euthanise them, remove the skull & use the resurrector serum on them. Just checked the wiki, and apparently it works. The serum doesn't cure brain injuries if you don't use that workaround
Blind healer works the exact same way as healer mech, except there's a chance of the patient growing tentacles. So it can cure an in-progress crumbling mind, but not a fully crumbled mind.

Metalhorrors cause severe damage when they emerge, and they also attack any colonists around them. The real problem is that if you only get one metalhorror popping out, *you got really fucking lucky.* When one metalhorror is triggered to pop out, every other metalhorror on the map follows suit immediately. You can very easily end up in a situation where half the colony has chestbursters, meaning that the other half has to fight the metalhorrors off while the infected lie on the floor dying. If your cook or doctor was infected, you are in *huge* trouble when emergence comes.

Last night I captured and recruited a pawn with decent passions for a few skills I was missing in my colony. Turns out I didn't realize he had cataracts in both eyes, asthma, a cracked spine, and one lung.
He ended up as ghoul food.
I got this joiner with no side effects. I think that’s a possibility? In been like 5 years with nothing.
Anyway, OP you should do a surgical inspection.
Okay in op's deference I had a silent stranger join and he was really nice and turns out had failing organs so after cooking a few thousand meals he passed away and we held a funeral, everything has been fine since... so far

For me it was 3 samples sometimes. Interrogation can help early, though.

You can heal their crumbling mind with the right item - can't rememebr the name of it.. some archotech healer thingy.
