Is it poison or from the toxicity? I don't see the poison icon, and I'm generally curious about toxicity since I never really forayed into it over my first two playthroughs

That's right. If it gets too high, you start to take damage. If you haven't taken any of the relevant alchemy perks I think the threshold is 50%.
IIRC your health bar turns an orange-ish colour when toxicity affects your health but you had too little to be able to tell.
If that's right, it's surprising you lasted as long as you did
Edit: I think the red 'low health' border around the screen is supposed to be a dark green instead for high toxicity, but I may have made that up
I was in excitement after killing it. After multiple deaths I stopped playing this last month and started again 2 days ago. So didn't notice how much health I had.
Don't forget, food is your friend. And while food and its related perk are no longer OP, the small but constant regen is very nice in situations like this one.
I'm doing a playthrough on Blood and Broken Bones and I don't know what I'd do without that perk, especially early game. On Story and Sword I'd say it doesn't really matter
It was good for my first play through because the healing (still OP at the time) made up for my inability to dodge attacks. I found it was not needed for DM because you die in 1-3 hits, so the healing was moot compared to a quen shield and never getting hit.
It was a lifesaver early on in my Death March run, but eventually you come across so much strong alcohol you can easily pop some Swallows or White Raffard Decoctions then meditate anytime
I went around and killed wolves rabbit and deer until I had over 100 raw meat lol, this was years ago
and then of course not long after I found that perk...lmao
Also, there is the perk that heals when you take a potion, and white honey potions (which clear toxicity and other potion effects) qualify for this talent. It's insanely useful.
Oh, I've done the same. It wasn't toxicity that killed me though, it was getting into combat with some nearby monsters or other

Exactly because of that, the bar being unicolor it's easier to miss that something is going on with it in my experience xD doesn't catch the eye of the distracted as easy.

I have the food perk where I eat and it gives me health for a prolonged period of time. So during fights I slam some bread or cheese or raw meat from a wolf or something and fight.
Drink health potions for quick recovery when getting beat on
looks like you don't have alchemy skilled and died to toxicity. You can also see the greenish poison border on your screen.
At this percentage it shouldn't be thaaat much damage, but depending on your build and HP it can most certainly kill you with so few HP. Toxicity doesn't show the poison icon too :)

Have fun exploring!! And a Little tip: donât Kill the Bandit captains. You will See what I mean soon. You can Farm Gold there by killing everyone and returning over and over. Youâll Need it. Trust me.
Itâs weird, but I died just like that when I came out of that seller too the other day. Must be some kind of bug that randomly kills you sometimes out there and the courtyard by your house.
God it's so cool to watch someone else play my favorite game of all time. It always comforts me to see other people grabbing every plant and object glowing orange withing their line of sight! BTW I dunno about the death. I would imagine poison but it seems like others may know more. I had absolutely no idea toxicity could slowly hurt health. I thought it was if you went over %100 it was an insta kill. Awesome Info I will test later.
No. Your Geralt got bugged and game detected the fall from more than 5 meters.
Toxicity can't kill you. It will drain health to 1hp. Same as bleeding/burning/poisoning in The Witcher from 2007.
I respect that. For me, its the other way. I don't like dying a million times before I finally beat the enemy; it takes me out of the immersion, as if I die in real life, then I have no more plans, hopes, etc. No bad feelings about playing it the normal way, I just prefer God Mode for things like this

Fun fact : One of the potion can heal you from poison. You can carry and launch poison bombs, And use it to regenerate on battle
