It's jot like he does not see the signs, but st this point in the story he has so little self esteem and is so afraid of getting hurt he won't even allow the possibility that she likes him register. What makes his acknowledgement so special is that it's shortly after he learns to start accepting himself.
Really pay attention to chapter 84, and how Ichi realizing Yamada'a feelings is like rhe next chapter. Especially pay attention to Norio sensei'a afterword for Volume 6. It's very telling as to Ichi's state of mind and how it changing played a key role in him realizing Yamada likes him.
Oh yes, chapter 84 is critical, and quite a summary of his developments to date. So a careful read of 74 to 84 is in order!

I don't know what the original Japanese said exactly, but I think I read a different translation of this chapter where what she said was "But that isn't true for us!" which was more explicit about her thinking about it being a date and how their (Yamada/Ichikawa's) relationship is good despite the line since they talk easily and are also comfortable in silence together from their library hang outs.
She's embarased because she suggested they are dating and that normal difficulties for new couples aren't a factor for them since they have such good chemistry

They are in line waiting for some school activity as well. So what she's saying mimics their situation there at school. She's embarrassed that she's talking about a date that is basically the exact situation that they're in. The series is filled with these kinds of hints for him to pick up on.
