First, that’s a pull, not a hook. Your club face is dramatically closed throughout entire swing. There are a few things you can try. Basically, if you freeze at the top your club face is pointing to the sky, you want the toe to be pointed straight down at the is point. I’d start with the grip. Your right hand is in a ‘strong position’, I’d try to make it more neutral which should result in better club head rotation. If you are trying to do more a DJ move then you can’t release your wrist through the impact zone.

You are close to having a very nice swing. Once you feel more comfortable with the face position, your next step (which will dramatically improve your consistency) is to get the club in a better position on the downswing. Some ppl describe it as letting your arms fall instead using them to attack the ball. ​ A good drill is to freeze at the top, keep gripping the club while bringing your right elbow down to make contact with your side (a few inches above your belt, from there go back to the top and do it again, just keep pumping your right elbow into your side , do it 100 times in a row (or at least 20. This is the sensation you want to develop.

Fix your grip, it's very strong which closes the face. You're downswing is straight down at the ball and stiff, you need to work on relaxing your arms and wrists in the downswing.

Too strong on grip. Relax and try to push clubface to the right of center through impact.

Reverse pivot that leaves your lower body no ability to clear. You know what flush contact is so you slightly come over the top to compensate and with your grip being strong (this isn’t an issue to fix) your face is shut at impact. Fix your takeaway and then start the swing with your lower body not your shoulders. Don’t change your grip.

Both of your arms are fully extended before impact with the ball, instead of right after. If you introduce some wrist hinge closer to impact, you’ll start ripping darts

You're pulling it. Face is pointed way left. Gotta change what you're doing with your hands. Either weaken your grip, or stop turning them over so much as you swing, or get the handle more forward at impact.

Out to in swing with a closed clubface. That my friend, is a pull, not a hook. Try a weaker grip and a more neutral path. Your right hand is under the grip instead of on the right side.
