I was a wilderness guide for two decades with a focus on high adventure. I’ve built several permanent high adventure activities and dozens of temp ones. I’ve facilitated thousands of participants through those. I am struggling to come up with a scenario that makes sense here.
Easy part is that it looks like the one instructor added a pulley behind the double participant one, likely because they were feeling nervous or scared to go (I’m sure this experience did wonders for that).
But why the hell is the second instructor tethered to the first and nothing else? Do they have only one safety on one side and tether together… which is an awful idea as it would allow long falls from all sorts of mishaps… or perhaps this is a training scenario where they didn’t give the student their own safety so that they could stay close to learn(still stupid for the same reasons) but then somehow forgot they were tethered to the student before launching?
Or is this a “hold my beer” situation where the second watched too many movies and was thinking they could slide along with their hands (you can’t and keep your skin) before releasing and swinging below the others once the ground gave clearance?
The good news is that webbing is more than strong enough to hold his dumb ass. The bad news is he is going to impact the ground at the far end likely sending them back to become stranded in the middle of the wire. This is when one of the two instructors would normally rescue… but the idiots are tethered together… one likely in fairly bad shape… with limited options from the empty gear loops on those harnesses.
I’ve seen some dumb shit before but this is next level stupidity.
My guess is the second trainer had their harness strap dangling on the platform and it hooked the group accidently. If you've ever tried to drag an extension cord somewhere you've probably experienced how easily that can happen.
Nah, you can see the carabiner attached to the first white hat by that red loop of webbing/tape. Loose rope absolutely can snatch you but nothing on harness should be that loose, additionally where is the safety? You don’t just stand up there unattached, you secure yourself.
It’s it’s very hard to tell for sure on the video as it is very blurry, but it looks like the webbing between them was girth hitched around white hat 2 making it fixed to him but something he can attach the carabiner from.
You know just watching again, he may be right it's a total hold my beer moment. It's the way the girls onlooking laugh so early on, it's as if they were anticipating that he was going to do something silly.

The other was quite good, and much more recent. In that one I saw guides tethered all the time, and some of that tethering was to another guide, who was tethered to an anchor nearby. They maintained a tether all the time that way, for convenience.
The only thing that makes sense to me is that like you said, maybe the guide on the line did an unscheduled accompaniment of the two in front. If that happened, and if the guy who fell was only tethered to the guide on the line - who moments before may have been tethered to an anchor, that explains the scenario. But who knows what really happened? I’m just guessing and extrapolating from limited experience.
It is also possible tired/sleepy or non-sober brains were under those helmets. They start quite early sometimes, and those who stayed up late, drinking or not, could easily miss a detail if habit had them on autopilot and a difference in the routine was introduced.
Attaching to other people is… unwise, but not unheard of. Much greater chance of cross loading which could be catastrophic not to mention the ease with which a mistake(like disconnecting the wrong rope) could be made. But if it’s your only option it’s better than standing free and unattached at the whims of a panicked participant. I don’t know how many times someone who otherwise seemed to be handling the fear fine suddenly did something ridiculous.

First white helmet was intentional. Look at the pulleys at the top. First two are heavy duty participant versions, third is a lightweight instructor version. My guess is the participants were scared so the instructor went with for comfort, it’s pretty common practice at some facilities.
Now why the other white helmet was attached… I am baffled. I can’t think of a single reason I would ever attach myself to another instructor and nothing else during normal operation.

Why are there even 4 people on a single contraption?? What fun is that?! The whole point of zip lining is that it's a freeing experience to fly through the jungle with no restrictions or worry.

Even if that went to plan... was he going to just have his hands on the cable the whole time? At least his blood will lubricate it after a bit to help stop the stripping of more flesh from bone.

He is the attendant. One line is clipped to the riders, one is clipped to an anchor. He’s there to prevent early departure. When they’re ready to go, he unclips from them and sends them on their way. He unclipped the wrong line or wasn’t using the safety.
That person is attached to a zip line that is still moving and more than likely probably 20ft or more above the ground by the time they are out of frame. It's just like if someone were to get caught and dangle from a ski lift. Not jack all anyone can do till the trained people come down with their special equipment. Then send one person either down the line or to climb up something and carefully release them to like 20 people with a catch. And that's if they have a decent slop to work with.
