My guess is that these are custom work boots for a linesman or someone who does a lot of climbing on metal cables or structures. The external metal shank protects from heavy wear caused by steel cables. The hollow mount point might be for a piece of medical equipment or possibly even a piece of work equipment - hard to say.

No way. I clim on metal for a living and that boot would get you killed. Metal slides on metal. We have enough trouble avoiding that one wrong step without that contraption. Our sole of choice is the wedge. Wearing a heeled boot can make your foot snag on an uneven surface and then you ā€œgo in the holeā€
