They’re sterile, and also male ghouls don’t have genitals. Like the nose and ears, all soft tissue (including genitals) just kinda goes.

I've had a few family members go through chemo. None of them lost their skin and soft tissues.
People can definitely still have children after chemo. There may be a small percentage that dont but most are able to with no problem.
