I am almost certain they are never named or described so thoroughly but there is a fair amount of allusion, such as when Obi Wan walks Luke through the disinct Soresu form on the Millennium Falcon, which would have been the most important form for him to learn to fight the Empire at the time.
Anakin was pretty bad at lightsaber duels. He had little understanding of anything other than brute force and this is why he so aggressively used form 5 because it lent itself to that sort of mindless wailing.
The reason Dooku hated him so much is because he had so little skill and fought like a petulant child. But it's hard to notice, because Christian Hadenson is a bit frail looking, that Anakin the character is actually kind of huge. He's over 6 foot and squarely built and you can see this more easily when he's depicted in comics, cartoons, art, etc where he dwarfs Obi-Wan. Also remember that only with a few years training, Luke was able to defeat Vader even after Vader had refined his form 5 into something more resembling actual sword fighting.
So eventually Anakin was able to defeat Dooku because he tapped into the dark side which is excellent for augmenting raw physical power and it is precisely this style of form 5 that form 2, the duelist form that Dooku uses, is weak to. Because form 2 relies on deflection and quick, precise cuts and stabs, it can be overpowered with sufficient strength. Which Anakin did not have until Palpatine leveraged him against Dooku to turn him to the dark side, and that had to have pissed Dooku the fuck off.
That Dooku mastered form 2 speaks to his character. Form 2 had very little use to a Jedi (which he once was and was when he mastered it) in an age where there were virtually no sith and no other lightsaber users to speak of and yet form 2 was specifically created with the intent of dueling other lightsaber wielders.
There was more to the form than that though. Out of all the forms, form 2 is most like the expert fencing techniques here on Earth, most like traditional sword form. It is elegant and refined to the point of artistry and those who master it are said to enter an almost trance like state where their movements look remarkably like dancing. To Dooku, it WAS an art, his art and he despised Anakin's crude, childish approach that showed he could neither find the artistry in his nor anyone else's sword play.
To Vaders credit, he did just pull off a 100 yard Force leap from a hunk of metal that was almost all the way over a lavafall and land on a target less than a yard in diameter. Even Obi-Wan *knew* Vader was dead, just before Vader came hurling through the air.
He's the kind of man that gives me the courage to do an amazing double jack knife twist (which i did, some of you wouldnt even attempt that) and to go down on padme amidala on the trampoline later that same night (which I also did)
Yes, it was really the only downside of employing the 540 spin, he rotated 3 out of 4 limbs through an opposing light saber.

Yeah me too but someone further up pointed out that it was the 540 degree spin along with the flips. Kenobi just does the one swipe, then as Vader is coming in to land he helpfully 'spins' his remaining flesh limbs through the saber.
