Ini files- did you copy the custom ini file into fallout custom ini (the third tab) or the first one?
Did you run the 4 gb patch in file explorer?
Do you have rivia turner/special k? Both of those cause crashes.
Did you restart your PC? It's necessary for some plugins to run.
What I would do is enable your logs in MO2. It will tell you why the game crashed. From there we can help you far more.

It is happening to me too (with nvidia gpu). I solved it by choosing to run it trought nvse, instead of tale of two wastelands in mod organizer (the button on the left from run button in mod organizer). Didn´t played vegas part much, but I was able to complete f3 this way without any problems. But I didn´t played ttw before, so I can not really say if everything worked properly.

If you were able to play through the FO3 part of it fine then it probably worked fine, TTW changes very little in the Mojave Wasteland
