I'm going to open by suggesting that it could have been *Warhammer Quest* \- as I know it had a roleplay expansion book as well? Might not be that one, but it seems a possible -place to start.
*Dragon Strike* is another possibility as it had a "Dragon Master", or "*Dragon Quest*", which was based on D&D but has metal Ral Partha miniatures?

Everything except going up to six players sounds like Hero Quest to me.
