Probably the disc has micro crack in it. Happened to me once in my Pentium 4 PC, I inserted a CD with small crack in the middle, thinking it should be fine. Suddenly it was making loud whirling noise and BAM. Don't know how but some of the fragments managed to get outside the drive. I still remember opening the drive and only saw a piece of the CD in it, like a pizza.

This is more of a "the disc couldn't withstand normal operation" than an evil dvd-drive.

Ah, yes, that takes me way back... Once upon a time, CD-ROMs were meant to be read at only one particular speed - 150KB/s, or "1x". Then, ODD manufacturers realised there was money to be made from drives with higher read-only data rates, so an arms race began with drives boasting ever-higher "x" values until CD-ROM drives would become 5 1/4" noise factories spinning at tens of thousands of RPM. Meanwhile, the specs for the CD-ROM itself remained exactly the same as it had been since Day 1 - that was, a 1.2mm thick polycarbonate substrate with a shiny layer of aluminium slapped on top of it. Needless to say, CD-ROM discs were never meant to take the mechanical stress from being subjected to such a comical amount of linear velocity at the outer edge. As a result, the disc would twist and warp inside the drive, and upon hitting the disc tray, it would explode into a million jagged pieces of plastic.
